THE QUALITY OF MEDICAL DOCUMENTATION AS AN INDICATOR OF PROFESSIONAL COMPETENCE

Abstract

This article provides a comprehensive analysis of the role and significance of medical documentation in the context of healthcare reform and the active digitalization of medical processes. The theoretical and practical aspects of maintaining medical documentation, its functional purpose, classification, and main forms − paper-based and electronic − are examined. Special attention is paid to the key functions of medical documentation, including clinical, legal, statistical, managerial, and scientific functions, which ensure a systematic approach to healthcare delivery and management of medical institutions. The importance of standardization of medical records, the use of international classifications, and unified documentation forms as factors in improving the quality of medical care and interprofessional communication is analyzed. The linguistic and stylistic features of medical documentation are highlighted, emphasizing the necessity of accuracy, logical consistency, and clarity in professional medical language. Particular emphasis is placed on the regulatory and legal framework governing medical documentation, ethical aspects of recording medical information, and the protection of patient confidentiality and personal data. The article substantiates the role of medical documentation as an essential tool of evidence-based medicine, quality and patient safety assurance, continuous professional development of healthcare professionals. The prospects for the development of medical documentation are considered in the context of implementing electronic health record systems, integrating national and international health information resources, applying digital technologies, and utilizing automated data analysis. The study concludes that improving medical documentation is a necessary condition for enhancing the quality of healthcare services, increasing the efficiency of healthcare system management, and advancing modern medical science.
